Creating new templates

Learn how to create templates using Eggmed's template editor.

Templates in Eggmed allow you to streamline processes by standardizing forms and documents you use with clients. You can create templates for various purposes, such as intake, assessment, and other types of forms, to suit your practice needs. Below, we’ll walk you through the steps to create templates and provide tips for customization.

1. From settings page

  1. Navigate to Settings > Templates from your Eggmed dashboard.
  2. Click on the “Create New Template” button.

2. From the client profile

  1. Go to the client’s profile and select the Activities tab.
  2. Click the “Assign Activities” button.
  3. In the slidout modal, navigate to the appropriate tab (e.g., Intake or Assessment).
  4. Click “Create New Form.”

Building your template

On the left side, there is a list of predefined elements you can use to create your template. They are categorized:

  • Display text: Includes text that cannot be edited by the person receiving the template (e.g., paragraphs, headings, lists).
  • Layout and media: Includes non-editable elements like images and tables.
  • Input fields: Editable fields for the person receiving the template (e.g., multiple choice, dropdown menus).
  • Contact information: Editable fields for phone numbers, addresses, etc.

"To get started, simply click on an element from the list on the left side of the screen. You can then enter your questions, notes, or any other relevant information in the designated fields. Alternatively, you can use the '+' button on the template page or the 'Add' button to create a new entry.

Additionally, you can format your text and add links using the buttons in the top bar. To preview your template, click on “Preview.”

Saving your template

1. Once your template is complete, select the appropriate type under “Save this template as:”

  • Choose Intake for forms used during the client onboarding process.
  • Choose Assessment for forms like GAD-7, PHQ-9, or PCL-5.
  • Choose Note-taking for forms you'd like to use during your sessions.

2. Click “Save,” and your template will appear under the selected category.

Tips for template creation

  • Standardize naming conventions: Use clear and descriptive names for templates to make them easy to identify.
  • Review for compliance: Ensure templates meet legal and professional standards, such as HIPAA compliance, where applicable.
  • Use existing templates as a starting point: Eggmed provides ready-to-use templates that you can customize to save time.
